'WEIGHT MATTERS' SUPPORT FOR INDIVIDUALS

Weight Matters so make a ‘small change and see a BIG difference’:

What do I need to think about ?

  • Checking where you are and what have become lifestyle habits.
    What you do you eat and drink?
    How active are you?
  • Changing what you choose to eat.
    What do you buy at the shops?
    How do you cook your food?
    Is it a Healthy Balance of Nutritious Food ?
    Are the portion sizes not too large?
  • Changing your physical activity levels
    Do you walk everyday ?
    Are there other ways to be active on your own?
    Is there a group to keep you going?
  • Changing your thoughts and behaviours which stop you sticking to eating well and being active.
    Do you have away of resisting external triggers?
    Do you have a substitute plan / way of thinking?

 

Ready to Change on your own? 


1. ‘Weight Matters’  R plan

Use the R plan with the support of the DOH Healthy Weight Healthy Lives Booklet
6 progressive brief interventions focussing on changing eating and activity habits
(Your Weight Your Health: How to take control booklet pdf and the R Plan pdf)                 

2. Weight Management – A Self Help Guide Workbook

This is a 12 step programme  to work through at your own pace. 
The booklet covers:-

  • Understanding the benefits and difficulties of making changes
  • Know what you want to achieve & planning your goals
  • Exploring current eating habits, the importance of keeping to a regular eating pattern and portion sizes
  • Reviewing your current activity habits
  • Keeping motivated
  • Explore shopping choices, cooking and who controls your food choices.
  • Why we eat and what triggers us to eat
  • How to overcome a lapse.

There are 2 ways of working though this guide;

i ) Going through the work book on your own
 The guide will be sent to you via the post once you have completed a registration form HYPER LINK FORM.

ii ) With ‘Step by Step’ support
This is based upon the above programme but is supported by regular email or telephone contact.  The steps are emailed out or posted to those who are do not have access to a computer.  Once you have completed a step you contact me for the next step and discuss any difficulties you have experienced.
