As of 1 April 2013, the Health Promotion Service's Stop Smoking Service will be moving to Cornwall Council from the NHS. All records, electronic and paper, that we hold will be transferring too. This information will continue to be stored and used as per the consent you signed when you were a client/patient of the service, following the strict requirements of the Data Protection Act (1998) and the Cornwall Council's Confidentiality Policy. If you believe we hold records on you, and do not wish them to transfer, please contact: Lynda Quee, Head of Service, Kernow Building, Wilson Way, Pool, Redruth TR15 3QE, 01209 313419 or email lynda.quee@cornwall.nhs.uk.
